Specialized psychological testing and wellness coaching with Dr. Fareed — serving neurodivergent and neurotypical children, adolescents, and adults across Pennsylvania and Massachusetts through secure, unhurried telehealth appointments.
Every referral begins with a question — about attention, memory, neurodivergent traits, personality, or surgical readiness. Dr. Fareed's role is to answer it thoroughly and translate findings into recommendations you and your providers can actually use.
In-depth assessment of cognitive functioning — memory, attention, executive function, language, and processing — following concussion, illness, or unexplained changes.
Inquire →Rigorous ADHD assessment for neurodivergent children, adolescents, and adults that distinguishes attention concerns from anxiety, mood, and overlapping presentations — never based on a single checklist.
Inquire →Gold-standard diagnostic evaluation for neurodivergent adults seeking clarity about autism spectrum conditions, with formal documentation and tailored recommendations.
Inquire →Comprehensive evaluation for personality disorders and complex presentations — integrating multiple measures to produce a nuanced, clinically useful picture.
Inquire →Pre-surgical psychological evaluations for adult bariatric candidates, meeting insurance and surgical center requirements with timely, thorough reports.
Inquire →Pre-surgical psychological evaluations for adults undergoing spinal cord stimulators and spine procedures, with clear findings surgeons and insurers can rely on.
Inquire →Structured, goal-oriented coaching focused on executive function, stress management, and integrating what you learn from testing into everyday life.
Inquire →Psychotherapy is not currently offered. If therapy is the right fit, Dr. Fareed is glad to provide referrals.
Dr. Fareed founded Signature on a simple belief: people deserve evaluations that are thorough, unhurried, and human. Too often, testing feels transactional — a score report handed over with little context.
With a decade of clinical experience, Dr. Fareed works directly with every client from intake through feedback. You'll receive an integrated report written in clear language, with recommendations that are specific, actionable, and grounded in who you actually are.
A straightforward path with a predictable timeline, so you can focus on the work itself rather than the logistics.
A complimentary 20-minute call to understand the referral question and confirm evaluation is the right next step.
A detailed telehealth conversation covering history, symptoms, and goals — the foundation every test battery is built on.
One to three secure telehealth testing sessions, scheduled flexibly around your work and personal life.
A 30–60 minute feedback meeting followed by a written report within two to three weeks of testing completion.
Dr. Fareed is currently licensed to provide psychological testing and wellness coaching via telehealth to residents of Pennsylvania and Massachusetts. Residency in one of these states at the time of service is required.
Dr. Fareed works with professionals across every field — educators, executives, first responders, attorneys, and more — offering goal-oriented coaching grounded in psychological science. Her deep expertise with healthcare professionals means she understands the unique pressures of clinical environments firsthand.
Coaching is not therapy — it's a structured, forward-looking partnership focused on concrete goals. The ultimate objective is to help you build sustainable systems that actually stick.

If something feels off — for you, your child, or someone you care about — an evaluation can move you from uncertainty to clarity. The deliverable is straightforward: a clear, readable report that lays out cognitive strengths, areas of difficulty, practical recommendations you can use at home, school, or work, and a formal diagnosis where one is appropriate.
Not sure if testing is the right fit? Start with a complimentary 20-minute phone consultation. If we're not the best match for what you're looking for, we'll say so and help connect you with someone who is.
The fastest way is to reach out by phone or email. We're a small practice, so messages are answered personally — typically within two to four business days. If you haven't heard back in that window, please try us again, as occasionally a message gets caught in a spam filter.

Online self-booking isn't offered because each evaluation is matched carefully to the client's needs before scheduling. The first call is short and informal — a chance to talk through what's going on and figure out next steps together.
Insurance is not accepted at this time. The practice operates on a private-pay basis.
Many testing practices choose this model for a few reasons. Insurance companies frequently restrict what can be tested, dictate which assessments are "medically necessary," and require a diagnosis to be added permanently to your medical record before they'll pay anything. That model often serves the insurer better than it serves the client.
The good news: many private-pay clients recover a portion of their costs through their plan's out-of-network benefits. After your evaluation is complete, we provide a superbill that you can submit to your insurer for possible reimbursement.
Yes. A superbill is provided automatically at the end of every evaluation, at no additional charge. It includes the procedure codes (CPT), diagnosis codes (ICD-10), date of service, fee paid, and clinician credentials — everything an insurance company needs to process an out-of-network reimbursement claim.
One thing to know: a diagnosis must be assigned for a superbill to be valid. If you'd prefer not to have a diagnostic code on record, you can still receive your full evaluation report — you just won't be eligible for insurance reimbursement.
Cost varies. Pricing depends on the type of evaluation, the scope of testing required, and how the referral question evolves during the process. A brief targeted evaluation costs less than a comprehensive neuropsychological battery, and we don't charge for tests we don't actually need to use.
Every prospective client receives a written cost estimate before any commitment is made. The 20-minute consultation is the right place to ask cost questions — and it's free.
Yes. The total cost is split across three payments rather than collected up front:
If you need additional flexibility around timing between intake and feedback, that's a conversation we're happy to have. Just ask.
Before testing begins, it's worth a phone call to your insurer to understand what reimbursement might look like. Call the member services number on the back of your insurance card and ask about:
If your insurer needs provider details, here they are:
Likely diagnostic codes (such as F90.2 for ADHD or F41.1 for Anxiety) can be discussed during your consultation if it helps your conversation with the insurance company.
One caveat: insurers usually only reimburse evaluations they consider "medically necessary." Coverage for academic, occupational, or general clarity testing tends to be more limited than for diagnostically-driven assessments.
Records stay with the practice and are protected under standard professional confidentiality. They aren't shared with anyone — insurers, schools, employers, family members — without explicit written authorization.
The exception: if you choose to submit a superbill to your insurance for reimbursement, a diagnosis code travels with that submission, and insurance companies aren't held to the same confidentiality standards a clinician is. For some clients, that's a meaningful reason to stay private-pay rather than seek reimbursement.
Most services are available for children, adolescents, and adults. A few are scoped to specific age ranges:
If you're unsure whether your situation fits, the consultation call is the best place to start. We'll talk through what's going on and confirm whether it's something we can help with.
Telehealth sessions take place on a secure, HIPAA-compliant video platform. You'll need a quiet, private space without interruptions, a stable internet connection, and a computer or tablet with a working webcam. A phone alone usually isn't enough for the visual portions of testing. Setup details and a link are sent in advance of the first session.
For evaluations involving children, a parent or caregiver typically helps with setup and may join part of the intake interview. The actual testing is conducted directly with the child, with check-ins between sessions as needed. Some testing instruments are administered verbally, others use on-screen visual material, and a few involve completing rating forms between sessions.
The full evaluation usually spans two to three testing sessions plus a separate feedback meeting. Total time varies based on what's being assessed.
Psychology licensure is regulated state by state. Services can only be provided to clients physically located in PA or MA at the time of each session. This means brief travel out-of-state during your evaluation can complicate scheduling.
If you live elsewhere, please reach out anyway — we're glad to suggest qualified colleagues in your area who do similar work.
